To facilitate planning and scheduling, please fill out this form for all field trips and activities that require more than the allotted $600 SSP budget. Note: You must submit a request form for all field trips requiring bus transport.

Will you be requesting bus transportation?

Yes

You must submit a request for all field trips/activities requiring bus transport. You do not need to submit a price quote for the bus transportation. The Office of the FYE will work with the bus company to schedule transportation and complete payment for transportation services.

Do you need assistance with making arrangements (i.e. group ticket sales, reservations, etc.)? Please explain. Note that unless specified, all transportation arrangements will be made by the FYE Office.
